The clinical distinction from postural kyphosis is rigid inflexibility: a teenager with Scheuermann's kyphosis cannot fully straighten their upper back even when actively trying.
Age-Related Hyperkyphosis
Age-related hyperkyphosis is driven by a combination of factors: progressive disc degeneration in the thoracic spine, vertebral compression, and weakening of the posterior spinal musculature that can no longer adequately support upright posture. The result is a progressive forward lean that worsens over years and decades.
